The candidate applying for the SEATA Memorial Graduate Scholarship has requested a letter of recommendation from you as part of the application process.  Please complete this form and submit as indicated below.  This information is forwarded directly to the District Nine Scholarship Committee.  Please be as honest and objective as possible.  The applicant must request a letter of recommendation from someone other than the supervising athletic trainer.  The letter should be from a person felt to be best be able to judge the applicant's qualities and abilities in the areas of academics and social involvement.
